RESPONSIBILITIES

Business Profile Description

With a capacity of around 430 beds, Peacehaven Nursing Home provides compassionate, individualized, holistic care to all without discrimination. We offer a comprehensive gamut of services to care for the sick, frail and elderly who have physical or mental disabilities with minimal or no appropriate caregiver support at home.

The Home has 14 separate Resident Living Areas (RLAs), each designed as an individual home-like environment. Hope RLA is specially designed to meet the needs of relatively independent residents with dementia.

Peacehaven Nursing Home has also been awarded the bizSAFE level 3 status which affirms our commitment to safety and health standards at the workplace.

Join Peacehaven Rehabilitation Services and experience one of the most comprehensive environments in the ILTC sector. Here, you will have the opportunity to support clients throughout their entire rehabilitation journey. Working alongside a multidisciplinary team-including Physiotherapists, Occupational Therapists, Speech Therapists, Dietitians, Therapy Aides and Assistants-you will provide meaningful rehabilitation services to a diverse population.

Peacehaven offers a variety of rehabilitation settings where physiotherapists play a key role in delivering quality physical rehabilitation with personcentred care in mind:

Key Responsibilities

Community Rehab (Active Rehab and Maintenance Rehab)

We support clients referred from acute, subacute, and community care who can benefit from active rehabilitation to improve their physical and mental wellbeing. Working closely with therapy aides, we aim to empower each client to remain active and independent within the community.

Key Responsibilities - Community Rehab

• Conduct initial assessments and regular reviews in accordance with clinical guidelines.
• Prescribe and conduct individual and group therapy programmes.
• Review and adjust rehabilitation exercises carried out by therapy aides.
• Collaborate with other healthcare professionals to deliver holistic, clientcentred care.
• Participate in continuing education, training, and research initiatives when available.

Peacehaven Day Centres

3 Day Centres Across the Community Physiotherapists support clients through engaging physical and cognitive programmes aimed at maintaining functional abilities for independent living. Working with therapy aides, you will plan and supervise exercises and activities tailored to each client's needs.

Key Responsibilities - Day Centres

• Conduct initial assessments for new admissions.
• Perform 6monthly care plan reviews and programme evaluations.
• Prescribe and deliver individual and group therapy activities.
• Review and update therapy programme schedules with proper documentation (InterRAI).
• Work closely with multidisciplinary teams to ensure holistic care.
• Contribute to multidisciplinary case discussions and ongoing staff development.

Peacehaven Nursing Home

Capacity: 430 beds Peacehaven Nursing Home provides compassionate, individualized, and holistic care to residents with physical or cognitive impairments who lack appropriate caregiver support at home. Physiotherapists work within a multidisciplinary team to help residents maintain their functional abilities as much as possible.

Key Responsibilities - Nursing Home

• Conduct assessments for new residents.
• Carry out 6monthly reviews and updates to care plans.
• Conduct, prescribe and supervise multi diagnosis therapy programmes for individuals and groups.
• Develop series of physical enhancing exercises to maintain residents wellbeing.
• Ensure accurate and timely documentation using IngGot and InterRAI.
• Collaborate closely with healthcare professionals to provide holistic care.
• Participate in continuing education, training, and research opportunities.
• Liaising closely with other healthcare professionals to encourage a holistic approach to Care
• Participate in multidisciplinary case discussions
• Participate in continuing education, training and research activities when opportunity arises

QUALIFICATIONS

• Bachelor degree / Diploma in Physiotherapy
• Full registration with APHC

Preferred Candidate Attributes

• At least 1 year of experience in the ILTC sector (preferred)
• Strong teamwork and communication skills
• Passion for working with older adults and communitybased clients

Working Hours

Under normal circumstances the hours of duty will be as follows:

8.30am to 5.45pm (Monday to Friday, 45 minutes break inclusive)
