Job Description:

Dispensing and educating patients on their medications, medication review and reconciliation, therapeutic interventions and recommendations, multidisciplinary rounds and collaborative prescribing for qualified persons.
Recommend and monitor drug therapy to achieve desired outcomes.
Provide drug information to other healthcare professional and patients.
Identify, resolve and prevent drug-related problems.
Provide pharmaceutical care, working collaboratively in a multidisciplinary team in various clinical settings, including outpatient, inpatient and community settings.
Ensure pharmacy policies and procedures are implemented consistently.
Other ad hoc duties assigned
