We are looking for a motivated Occupational Therapist to provide evidence-based rehabilitation services that support individuals in improving their independence and participation in daily living activities.
The successful candidate will work closely with a multidisciplinary healthcare team to assess patients functional abilities, develop treatment plans, and implement therapeutic interventions to enhance recovery and quality of life.
Manage assigned caseloads involving inpatient, outpatient, and community-based rehabilitation cases.
Conduct comprehensive occupational therapy assessments and develop individualised rehabilitation plans.
Implement therapy interventions and monitor patient progress.
Perform regular reassessments and update treatment plans accordingly.
Support discharge planning and transition of care where appropriate.
Ensure proper documentation of patient assessments, treatment plans, and therapy progress.
Communicate effectively with patients, families, and caregivers regarding therapy goals and rehabilitation progress.
Provide caregiver education and training to support patient rehabilitation and functional independence.
Encourage active participation of patients and caregivers in therapy programmes.
Participate in multidisciplinary team meetings, case conferences, and care planning discussions.
Work collaboratively with healthcare professionals to provide holistic patient care.
Ensure proper handover of cases during leave or shift transitions.
Maintain accurate clinical records and documentation in accordance with organisational policies.
Ensure safe use and maintenance of therapy equipment.
Participate in quality improvement initiatives and incident reporting when required.
Assist in collecting statistical data and reports related to therapy services.
Adhere to healthcare regulations, patient confidentiality requirements, and relevant clinical guidelines.
Participate in continuous professional development (CPD) and training programmes.
Stay updated on current occupational therapy practices and treatment approaches.
Contribute to departmental education activities and knowledge sharing.
Diploma, Bachelor's or Master's Degree in Occupational Therapy recognised by the Singapore Allied Health Professions Council (AHPC).
Registered with AHPC with a valid practising certificate.
Valid BCLS + AED certification.
Preferably 1-2 years of relevant occupational therapy experience in a hospital, rehabilitation centre, or nursing home setting.
Experience in community care or aged care settings will be an advantage.
Good communication and interpersonal skills.
Ability to work independently and within a multidisciplinary team.
Strong critical thinking and problem-solving skills.
Good organisational and documentation abilities.
Basic computer literacy (e.g., Microsoft Office)
