About the Role
We are seeking a dedicated and compassionate Senior / Occupational Therapist to join our multidisciplinary team. You will play a key role in delivering high-quality, patient-centred rehabilitation services across inpatient, outpatient, and home care settings.
Key Responsibilities
. Deliver professional occupational therapy services across assigned caseloads (inpatient, outpatient, and home-based cases)
. Provide therapy coverage for assigned ward(s)
. Conduct comprehensive assessments and develop individualised rehabilitation plans
. Perform reassessments (every 3 months for active rehab, 6 months for maintenance cases)
. Monitor patient progress and conduct weekly treatment reviews
. Plan and support discharge processes for patients
. Ensure proper documentation of assessments, treatment plans, and progress updates
. Participate in case conferences, multidisciplinary team meetings, and family discussions
. Collaborate closely with physiotherapists and other healthcare professionals in care planning
. Ensure proper handover of cases during leave coverage
. Apply evidence-based practices and sound clinical reasoning in all interventions
. Conduct risk assessments and ensure patient safety during therapy sessions
. Educate and engage patients, families, and caregivers to support rehabilitation outcomes
. Provide caregiver training where required
. Ensure safe use of therapy equipment and report any incidents or defects promptly
Requirements
. Master/Bachelor/Diploma in Occupational Therapy or equivalent recognised qualification by AHPC
. Fully registered with the Allied Health Professions Council (AHPC), Singapore
. 2-3 years experience relevant experience in hospital or nursing home setting
. Strong clinical reasoning and case management skills
. Ability to work independently as well as within a multidisciplinary team
. Good communication and interpersonal skills
