Job Overview
The Occupational Therapist will deliver professional and patient-centred therapy services across assigned caseloads, including inpatient, outpatient, and home care. The role involves comprehensive rehabilitation planning, clinical assessments, and collaboration within a multidisciplinary team to ensure optimal patient outcomes.
Key Responsibilities
Clinical Assessment & Care Planning
- Conduct initial assessments and develop personalised rehabilitation plans
- Perform regular reassessments (every 3 months for active rehabilitation and 6 months for maintenance cases)
- Establish individualised therapy goals and review rehabilitation plans on a weekly basis
- Provide timely updates on treatment progress and support discharge planning
Therapy Delivery & Case Management
- Manage assigned caseloads and provide therapy coverage for designated wards
- Ensure proper follow-up of patients throughout their rehabilitation journey
- Conduct rehabilitation case management rounds together with OT/PT teams
- Contribute holistic rehabilitation input during case discussions
Multidisciplinary Collaboration
- Participate in case conferences, team meetings, and family discussions
- Work closely with healthcare professionals to ensure coordinated and effective care
- Ensure proper handover of cases during leave or coverage periods
Patient & Caregiver Engagement
- Communicate effectively with patients and family members to support rehabilitation goals
- Encourage therapeutic carryover beyond therapy sessions
- Engage clients, families, and caregivers in treatment planning and progress reviews
- Provide caregiver training in a safe and professional manner when required
Safety, Compliance & Documentation
- Adhere to clinical guidelines and apply sound clinical reasoning in all interventions
- Maintain accurate and timely documentation, including assessments, progress notes, and treatment updates
- Conduct risk assessments and ensure appropriate safety measures during therapy sessions
Equipment & Quality Management
- Ensure safe and proper use of therapy equipment
- Report any equipment faults, incidents, or complaints to the relevant authority
Evidence-Based Practice
- Apply standardised, evidence-based assessment tools and treatment approaches
- Continuously monitor, evaluate, and adjust therapy interventions within a multidisciplinary framework to optimise outcomes
Job Requirements
- Degree in Occupational Therapy from a recognised institution
- Fully registered with the Allied Health Professions Council (AHPC), Singapore
- Relevant experience in rehabilitation, hospital, or aged care settings preferred
- Strong clinical reasoning and case management skills
- Good communication and interpersonal skills
- Ability to work independently and collaboratively in a multidisciplinary team
