Job Description

Brief Summary

Join a dedicated team as a Social Worker, where you'll play a crucial role in supporting patients and their families through various challenges related to their health. This position offers the opportunity to make a significant impact on the lives of those in need.

Responsibilities

Conduct interviews and provide counsel to patients and families to address medico-social and financial challenges. Collaborate with medical staff, nurses, and other healthcare professionals on comprehensive resident management plans. Coordinate with external organizations to fulfill the needs of residents and their families. Apply for financial assistance programs, such as Medifund, for eligible cases. Maintain detailed electronic records and statistics on individual residents, including those with specific challenges. Engage in fieldwork by visiting residents in various settings to resolve their social and financial issues. Prepare reports for management and relevant agencies as needed. Stay informed about government guidelines regarding healthcare and social services in the community. Develop and initiate new support services for former residents and caregivers. Assist in departmental workload distribution and supervise administrative staff. Participate in staff training and orientation, as well as internal or external committee activities.

Requirements

Recognized Degree in Social Work or a Postgraduate Diploma in Social Work. Registered with Singapore Association of Social Workers. Minimum of 3 years relevant experience in medical social work, with a preference for experience in the Intermediate and Long-Term Care (ILTC) sector.
