Job Description

Company Description Apex Harmony Lodge (AHL), founded in 1999, is a non-profit organization and the first purpose-built lodge in Singapore dedicated to persons living with dementia. Guided by a clear vision and mission, AHL reframes dementia as a unique life phase, focusing on strengths and person-centered care so that residents can live meaningfully. The Lodge offers integrated dementia care, including long-term residential services across the care spectrum. AHL also supports individuals with dementia and their caregivers in the community through various programs and services. The organization values each person with dementia as an individual with unique abilities, experiences, and contributions.

Key Responsibilities

1. Casework

  • Provide comprehensive case management, including care assessment and intervention

2. Financial Support

  • Facilitate financial assistance for new residents in line with MediFund guidelines
  • Conduct financial assessments and reviews to ensure continuous funding support for residents

3. Admissions & Bed Occupancy

  • Work with caregivers and referring parties to perform intake assessments
  • Coordinate the end-to-end admission process

4. Other Duties

  • Networking with healthcare facilities, social agencies and community partners
  • Active member of relevant committees, including Pandemic Response Team
  • Provide caregiver support and strengthen engagement with next-of-kin
  • Carry out ACP/PPC documentation

Requirements

  • Degree in Social Work
  • Registered or eligible for registration with the Singapore Association of Social Workers
  • Minimum 2 years of relevant experience preferred
  • Familiarity with Medifund, MFEC, PGDAS, IDAPE, Eldershield/Careshield, and Medisave
  • Strong administrative, writing, and conceptualization skills
  • Compassionate, mature, and team-oriented with excellent interpersonal skills
  • Knowledge of medical social work
  • Talents in wellness or creative facilitation are a plus
