Job Description

Key Responsibilities

  • Conduct joint home visits with Occupational Therapists (OTs) to assess clients psychosocial needs, goals, motivations, and barriers to community reintegration.
  • Co-develop and review client-centred care plans with OTs, integrating functional, psychosocial, and community participation goals.
  • Provide case management, motivational interviewing, caregiver support, and assistance with environmental or home modifications where required.
  • Develop and oversee the Befriender Buddy volunteer scheme, including volunteer recruitment, onboarding, training, matching, and ongoing support.
  • Coordinate ReAblement Care Associates (RCAs) and Befriender Buddies to support clients goals and community participation.
  • Facilitate social prescribing and linkage to community activities, groups, and services, including accompaniment where needed.
  • Support discharge planning, follow-up arrangements, and continuity of care with clients, caregivers, and community partners.
  • Facilitate interdisciplinary case discussions with OTs, RCAs, and Befriender Buddies to ensure coordinated, person-centred care.
  • Maintain timely documentation and support programme reporting, monitoring, and achievement of programme outcomes.
  • Collaborate with ICCP and community partners to strengthen continuity of care beyond the programme.

Qualifications

  • Degree in Social Work or Graduate Diploma in Social Work.
  • Minimum 2 years of relevant experience, preferably in Medical Social Work, community care, rehabilitation, or home-based services.
  • Experience working in interdisciplinary teams and coordinating with community partners.
  • Strong interest in supporting functional recovery, community reintegration, and caregiver/family well-being.
  • Good communication, engagement, case management, and coordination skills
