Medical Social Worker

Role & Responsibilities

. Conduct psychosocial and financial assessments for patients/clients and their caregivers

. Provide direct counselling, emotional support, and psychoeducation to patients/clients and their families to facilitate coping with illness, grief, loss, and life transitions.

. Collaborate with the multi-disciplinary team (MDT) to develop individualized care plans and facilitate family conferences to support communication and shared decision-making.

. Act as an advocate and liaison for patients/clients and their families, liaising with internal and external stakeholders, community partners, and government agencies to ensure access to necessary resources and services.

. Proactively risks to patient/client well-being and intervene promptly and appropriately to mitigate potential harm.

. Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans, and progress notes in accordance with professional standards and organizational policies.

. Actively participate in and contribute to departmental and organizational quality improvement initiatives, service development, and program evaluation to enhance patient/client outcomes and service delivery.

. Uphold the highest standards of professional social work ethics, confidentiality, and practice as guided by the Singapore Association of Social Workers (SASW) Code of Ethics.

Qualifications, Competencies & Experience

. Master/Bachelor/Post Graduate Diploma in Social Work

. Preferably a registered Social Worker with the Singapore Association of Social Workers (SASW)

. Good team player with positive attitude, caring and compassionate individual

. Good interpersonal skills, able to communicate people at all levels such as patient's families, government agencies and other professionals

. A quality service mind-set

. At least 3 years of relevant working experience in a healthcare or eldercare setting

. Familiarity with dialects will be an advantage

. Advance Care Planning facilitator would be an advantage

. Inclination towards palliative care and/or community case management will be an advantage

. Ability to speak and write effectively in English.
