Job Responsibilities
- Support elderly patients and their caregivers with:
- Psychosocial and financial support
- Casework and counseling services
- To facilitate successful integration back to the community by:
- Collaborating closely with the multi-disciplinary team
- Implementing a comprehensive and coordinated care plan
- Implementing initiatives and support programmes that enhances the care and support of elderly patients and their caregivers within the community
Job Requirements
- Degree and postgraduate diploma in Social Work.
- Must have an interest to work with the elderly in a healthcare setting
- Ability to communicate with vernacular languages speaking clients. (in order to provide psychosocial, financial support and assistance to Vernacular Languages Speaking Clients.)
