Conduct comprehensive psychosocial assessments and develop intervention plans to address residents emotional, financial, and social needs in alignment with the care plan.
Facilitate advance care planning (ACP) and financial assessments to help residents and families understand medical decisions and access appropriate financial support.
Perform crisis and risk assessments (e.g., self-harm, violence, family conflicts), and intervene to maintain a safe, supportive care environment.
Plan and coordinate discharge processes, linking residents and families with community services and ensuring a smooth transition back to home or community settings.
Provide individual and family counselling to build resilience, strengthen coping mechanisms, and enhance overall psychosocial well-being.
Participate actively in multidisciplinary team discussions to integrate social perspectives into holistic resident care.
Maintain accurate case documentation, reports, and data entry in client management systems, ensuring confidentiality and compliance with regulatory requirements.
Support departmental projects and community engagement initiatives to promote psychosocial care and resident empowerment.
Job Specification
Degree in Social Work or Postgraduate Diploma in Social Work from a recognised institution
Registration with the Singapore Association of Social Workers (SASW) is preferred
1 to 2 years of relevant social work experience in healthcare, eldercare, or community settings
Experience in casework, counselling, or care coordination preferred
