Medical Social Worker (Home Care)

Responsibilities

• Conduct comprehensive biopsychosocial and financial assessments for clients and caregivers, identifying medical, functional, psychosocial, and financial needs. Develop tailored care plans and coordinate follow-up actions to effectively address identified needs. Collaborate with a multidisciplinary team, including healthcare and community partners, to ensure seamless care transitions and continuity. Provide emotional support, direct counselling, and psychoeducation to clients and caregivers dealing with illness, grief, loss, and life changes. Perform crisis intervention and facilitate timely referrals to specialized services for complex or high-risk cases. Conduct home visits independently or in collaboration with the Home Care team as appropriate. Ensure accurate, comprehensive, and timely documentation of assessments, interventions, care plans, and progress notes according to professional standards and policies. Engage in departmental quality improvement, service development, and program evaluation activities. Support outreach and educational initiatives to enhance awareness and increase enrolment in programs. Participate in ongoing training, supervision sessions, and casework discussions for professional development. Undertake additional tasks as assigned by reporting officers.

Requirements

• Degree in Social Work or a Graduate Diploma in Social Work. Registration with the Singapore Association of Social Workers. Relevant experience in means testing and a healthcare setting is preferred. Familiarity with local dialects is preferred.
