Responsibilities
- Provide counselling service to patients and caregivers
- Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
- Assist the patient to understand the diagnosis and implications on ADL functioning
- Walk through with patients and caregivers and provide emotional support and motivation for them
- Manage patients expectations and set realistic and achievable goals
- Arrange for interim care or make referrals for long-term step-down facilities when necessary
- Participate in ward rounds and case conferences to represent patients social, psychological and economic circumstances.
- Undertake projects and any other duties as assigned by the Management
Requirements:
- Master/Bachelor/Post Graduate Diploma in Social Work
- Registered Social Workers with the Singapore Association of Social Work (SASW) - will be an advantage
- Compassion and care for patients
- Good team player with positive attitude, caring and compassionate individual
- Good interpersonal skills, able to communicate with people at all levels, such as patients families, government agencies and other professionals
