You Will Be Responsible For

• Providing counselling, emotional support and psychosocial interventions within your professional scope
• Developing behavioral support plans and non-pharmacological interventions with the care team
• Supporting residents through emotional, behavioral, cognitive and adjustment challenges
• Assessing financial needs and facilitating assistance applications, including MediFund
• Conducting relevant assessments and developing person-centred intervention plans
• Supporting Advance Care Planning and palliative care discussions, where appropriate
• Working with hospitals and referral partners on intake assessments and admissions
• Coordinating care and connecting residents and families with relevant support
• Contributing to case conferences, care planning and service improvements
• Using assessment and programme data to evaluate and improve support
• Facilitating support groups, psychoeducation and therapeutic activities
• Maintaining timely, accurate assessments, case notes and reports
• Collaborating across disciplines to deliver holistic dementia care
• Reviewing funding arrangements to support continuity of care
