You will be part of HPB's Integrated Outreach team, which aims to create a Health Living Environment (HLE) where we make healthy living as part of every day life in Singapore. We aim to do so by place making, activating communities and empowering community leaders to play a role in health promotion. This transformative project shifts HPB's role from traditional programme provider to community enabler, embedding health promotion into citizens lived environments across entire neighbourhoods. We are seeking a dynamic and strategic Manager/Senior Manager to spearhead community activation and drive sustainable behavioural change together with healthcare cluster (SingHealth, National University Hospital Systems, National Healthcare Group) and other community partners at the town level.

[What you will be working on]

As a core memberoftheHLEinitiative, you willbe responsible foroperationalising an innovative approach to community health that reaches the unreached and transforms neighbourhoods into health-promoting ecosystems. This rolerepresentsHPB's strategic pivot towards citizen-centric, place-based health interventions that integrate clinical care with lifestyle support anchored in local communities.You will develop skills in translating data insights into clear strategies and practical actions, data storytelling, stakeholder engagement and negotiation, project management, and cross-agency coordination.

Key Responsibilities

RegionalRoll out andCoordinationof HLE

  • Support the rollout of the HLE initiative within theassigned region(s),ensuring consistency with national direction while allowing forregional/local adaptations

  • Apply HLE standards (e.g. digital wayfinding standard, engagement playbook, and collaboration manual) during initiative rollout and guide grassroot leaders and community partners (e.g. AACs) in using these resources effectively

  • Utilise community health dashboards and local data insights to empower grassroots and community leaders inidentifyingpriority health needs and co-creating targeted interventions that address specific neighbourhood health gaps

  • Coordinateandsupport HLE implementation, including meeting facilitation and general project management tasks

Stakeholder Partnership and Capability Building

  • Work with grassroot advisors/leaders, clusters, and community partners to co-create and coordinate neighbourhood-level activities and strengthen community-led health agendas, building community ownership and optimising community spaces into vibrant, health-promoting environments

  • Deliver training sessions and provide practical support to grassroot leaders and community partners (e.g. AACs) in applying the playbook, message house, menu of actions, wayfinding guides, and dashboards

  • Support the team in collaborating with key healthcare partners in their planning and implementation of regional health promotion activities

Programme Activation and Targeted Outreach

  • Coordinate community and regional health programmes, ensuring they are relevant to residents and responsive to their needs

  • Utilise data to prioritise high-need groups, working with GRAs to apply targeted outreach models, outreach quadrants, and activation toolkits in local contexts

  • Connect residents to healthy living assets and programmes byenliveningthe lived environment and the Healthy 365 online-offline resident journey, through local mapping that guides them from discovery to sustained participation.

Monitoring, Feedback and Continuous Improvement

  • Track ground-level progress of HLE initiatives,consolidatefeedback fromresidents,localstakeholders, and share insights with the project team and data teams to strengthen impact measurement

  • Collate,utiliseand evaluate data from other sources where required, to create a comprehensive picture of the precinct's health profile and its target segments

[What we are looking for]

An ideal candidate will be acollaborativeleader with a passion for community healthpromotiontransformation and systems thinking. You should have experience in community engagement, partnership management, or public health programme implementation, with the ability to work effectively across diverse stakeholder groups.

While a degree in a specific field is not mandatory, candidates with backgroundsin business administration/management,public health/public policy/public administration, sociology/psychology, health sciences, communications/media, orrelated fieldoftenpossessrelevant skills.Trainedin project or account management is an added advantage.

The candidate should alsopossess:

  • Strong collaborative and interpersonal skills with the ability to build rapport with diverse stakeholders
