Homecare Senior Staff Nurse (West) #HJO

Job Responsibilities:

• Assess patients conditions and care needs during admission and throughout their care period

• Plan, implement, and evaluate nursing care based on patients clinical needs

• Monitor patients progress and document nursing care and outcomes accurately

• Administer treatments and carry out procedures as prescribed by physicians

• Work closely with doctors and allied health professionals to coordinate patient care

• Provide patient and family education on health management and care plans

• Ensure safe and effective delivery of nursing care while maintaining patient comfort and dignity

• Assist physicians during procedures and clinical rounds when required

• Maintain proper use of medical equipment and supplies

• Ensure a safe and clean environment for patient care and workplace safety

• Support care coordination and smooth transitions between healthcare teams

• Supervise and guide junior nurses, new staff, and nursing students when required

• Participate in improving care processes and nursing practices

Requirements:

• Registered Nurse with valid Singapore Nursing Board registration

• Relevant clinical nursing experience preferred

• Good clinical assessment and patient care skills

• Strong communication and teamwork abilities

• Proactive, responsible, and able to work independently
