Responsibilities
- Deliver home-based nursing care services requiring advanced nursing skills. Conduct caregiver training at clients residences. Anticipate clients needs and mitigate potential risks through proactive advice. Plan, develop, deliver, and evaluate individualized nursing care plans. Perform initial assessments and semi-annual reviews in a timely manner. Execute care plans and interventions appropriately and efficiently. Adhere to infection control measures as per established guidelines. Manage patient workload through effective planning and scheduling. Ensure timely and accurate documentation following home visits. Participate actively in multidisciplinary meetings and team discussions.
Requirements
- Registered with the Singapore Nursing Board with a valid practicing certification. Minimum of 4 years of continuous nursing experience in a medical or geriatric setting, with at least 2 years in recent practice. Experience in oncology or palliative care is advantageous. Strong communication and interpersonal skills, comfortable engaging with diverse individuals. Positive attitude toward learning and feedback. Adaptable, collaborative team player, responsive in various communication channels. Independent and highly reliable with consistent performance. Ability to make sound clinical judgments in a home-care environment. Keen interest in supporting end-of-life care for seniors. Willing to provide after-office-hour care for patients on a rotational basis (tele-consult). Good health to handle work-related tasks and mobility in the community.
