Locum Palliative Home Care Staff Nurse (Flexi office hours/ East) HGN

Job Descriptions

• Delivery of safe care and sound clinical decision making
• Assessing and documenting observations of clients condition at each visit
• Initiating/ reviewing nursing care plan based on assessment made
• Maintaining accurate nursing documentation
• Performing necessary nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing and etc.
• Conducting regular evaluation of care provided for home care services
• Provide caregiver training and education to client including families on home health care procedures and strategies
• Reporting of any problem incidents encountered in a client's condition and/or in handling clients and/or caregiver to Nurse Lead

Job Requirements

• Min Diploma and above in Nursing
• Valid SNB and BCLS
• Prior experience handling palliative patients
