Job Description

The Health Care Assistant is responsible for the delivery of daily clinical services to clients under the Home Care Services (HCS).

He / She will act professionally at all times and be accountable to the client and the organization to ensure that care is delivered in a safe and timely manner.

Job Responsibilities

  • Render care to clients in accordance with the Community Care Skills Standards Framework
  • Provide direct Home Care Services (HCS) to clients
  • Perform basic skills such as:
    • Vital signs monitoring
    • Assistance with Activities of Daily Living (ADL)
    • Assistance with Instrumental Activities of Daily Living (iADL)
    • Provide medication assistance/reminder
    • Escorting of clients
    • Transfer and use of mobility aid
    • Conduct appropriate exercise and activities
    • Provide service delivery according to service contract and maintains documentation accordingly
    • Be a good team player, building positive influence to team and motivated to learn and grow towards self-excellence
    • Participate in outreach and related activities
    • Communicate and relate effectively at the workplace
    • Perform any other duties as directed by supervisor and / or the management
Qualification & Skills

  • Minimum of Basic Nursing Aide Certificate

Relevant Experience

  • At least 2 years of relevant experience. Experience with community nursing is preferred.
