Health Advisor | Care Coordinator

Provideholistic and incredible care,empower patients and caregivers to take charge of their own health through technology,raise awareness and accessibility of available resourcesthat can be tapped on,nudging them towards action.

Strategic outcomes and goals include:

• Tobuild relationshipswithinternal (care team)andexternal stakeholders (patients and caregivers).
• Tocontribute to achieving clinical outcomesforpreventive health screeningsandreduce readmission and escalations to tertiary care, and, togather social determinants of healthpertinent toinfluencing social and health outcomes in positive ways
• Leverage on technologytoempower patients to improve health literacy and accessibility to resources
• Responsible forassisting in coordinating and providing patient care activities. This includes theuse of IT systems to track, perform and document established clinical indicators and care given to patient.
• You willperform basic parameter measurements,close preventive screening and vaccination gapsin relation tonational programmes/initiatives.
• You will alsoprovide basic health educationtoimprove patients knowledge on lifestyle management,social and physical activities through social prescribing, theuse of health monitoring devices(e.g.home blood pressure, glucose and weight monitoring, as well asadvise patients on vaccination requirements.)

Job Requirements

• Diploma in Health Promotion will be an advantage
• Prior healthcare experience will be advantageous
• Good verbal and written communication skills
• A team player with good interpersonal & communication skills
• Passionate to work in Healthcare sector
• Customer-oriented, a team player, a fast learner and serious with work
