Job Description

Overall Job Responsibility

Role

This role seeks to support the Future Care Planning Resource Centre's (FCPRC) efforts to develop the ecosystem of support for future care planning (FCP). These efforts include, but are not limited to, the following:

  • Support caregivers in FCP with key focus on complex case management.
  • Capability building of sector professionals to deliver FCP to their clientele.
  • Build collaborations with stakeholders/enablers to facilitate the FCP process.
  • Drive community engagement, public education and targeted outreach on FCP.
  • Support resource development for FCP.

You will also develop and implement relevant initiatives to fulfill the role of the FCPRC and achieve its goals.

Duties and Responsibilities

  • Practice Leadership
  • Provide professional leadership in Future Care Planning (FCP) practice across the disability sector.
  • Lead the development of practice standards, frameworks, tools and resources to support quality FCP implementation.
  • Provide consultation and guidance on complex cases involving caregiving arrangements, vulnerable families, support networks and future care decision-making.
  • Analyse emerging practice issues and recommend service and practice improvements
  • Facilitate Future Care Plans Development
  • Triage referrals on Future Care Planning.
  • Provide professional consultation and guidance on complex Future Care Planning cases surfaced by Care Planners and sector professionals and recommend appropriate intervention strategies and support pathways.
  • Facilitate development of future care plan cases unknown to SSAs.
  • FCP Sector Capability Development
  • Work jointly with the OIC of FCP sector capability development to address capability gaps and create tailored training solutions for FCP professionals.
  • Mentor and coach sector professionals and Care Planners in FCP practice.
  • Facilitate case consultations, reflective discussions and communities of practice.
  • Lead development of best practices in FCP.
  • Sector Collaboration
  • Serve as FCPRC's social work subject matter expect in FCP.
  • Engage sector leaders and practitioners to promote the adoption of FCP within social work practice.
  • Represent FCPRC at relevant professional forums, sector workgroups and practice networks.
  • Support FCPRC's efforts to develop and maintain partnerships with key stakeholders from FCP-related fields (legal, housing, financial planning, healthcare/ageing), ranging from government and private sector agencies to social services and community-based agencies.
  • FCPRC Operations
  • Work with the OIC of FCP resource development to develop FCP outreach and public communications materials.
  • Support the day-to-day running of the FCPRC where necessary.
  • Support Group-level projects or initiatives related to FCP, as assigned by team leaders.

Any other duties as assigned.

Qualifications, Knowledge And Experience

  • A degree holder with qualifications in Social Work / Psychology / Counselling and at least 5 years of casework, group work or community work experience in the social service / disability / healthcare / education sectors.
  • At least 2 to 3 years of hands-on experience in leading teams or supervising junior caseworkers and mentoring and developing social work practitioners.
  • Proficiency in case management skills, with the ability to conduct comprehensive assessment, analyse the needs and risk, plan and coordinate services, and monitor the progress of cases. Demonstrated experience in complex case consultation, supervision, practice leadership or capability building.
  • Strong relationship-building and interpersonal skills.
  • Proficiency in community outreach/engagement, with the ability to work effectively across diverse stakeholder groups.
  • Excellent communication skills – both written and verbal – to engage different stakeholders, drive alignment and get buy-in.
  • Familiarity with Person-Centered Practice (PCP) and/or legal instruments like Wills, Deputyship, Lasting Power of Attorney will be an advantage.
