Job Description
About the Role:
We are looking for an Executive / Senior Executive, Population Health & Digital Transformation to support initiatives aimed at improving chronic disease outcomes through population health management, clinician engagement, quality improvement and technology-enabled solutions.
The role will support the Diabetes Outcomes Improvement Programme, working closely with clinicians, clinics, internal stakeholders and technology partners to identify care gaps, implement improvement initiatives and develop digital solutions that enhance patient care and clinical outcomes.
Key Responsibilities:
- Analyse healthcare and patient data to identify care gaps, trends and opportunities for improvement
- Work with doctors and clinic teams to support population health and quality improvement initiatives
- Coordinate and monitor healthcare programmes, including tracking outcomes and progress
- Support the development and implementation of digital health tools for patients and clinicians
- Work with internal teams, IT and external vendors on system requirements, testing and implementation
- Prepare reports, programme updates and recommendations for management
- Support other population health and chronic disease initiatives as required
Requirements:
- Degree in Public Health, Healthcare Management, Nursing, Pharmacy, Life Sciences or a related healthcare discipline
- Relevant experience in healthcare operations, population health, quality improvement, healthcare analytics, digital health or project management would be advantageous
- Comfortable working with data and healthcare stakeholders
- Good communication, analytical and stakeholder management skills
- Organised, proactive and able to manage multiple projects and priorities
- Experience working with clinicians or healthcare technology projects would be an advantage
