What you'll be doing
- Lead the operationalisation of initiatives, ensuring alignment with organisational goals and health-social integration strategies
- Develop, implement, and monitor community health plans for assigned precincts, with a focus on preventive care and integrated service delivery
- Track progress, evaluate programme outcomes, and provide recommendations for continuous improvement across multiple focus areas
- Build and maintain strategic partnerships with internal teams, grassroots organisations, social service agencies, and healthcare providers to enhance community health and integrated care
- Provide leadership and guidance to programme teams, ensuring effective volunteer/staff engagement and delivery of initiatives
- Prepare and present reports, proposals, and updates to senior management and stakeholders
- Oversee secretariat support for meetings and ensure proper documentation of key decisions and action points
- Undertake additional projects or initiatives as assigned by the Reporting Officer or Head of Department
What we're looking for
- Bachelor Degree level in Healthcare/ Public Health/ Social Work or related studies
- 4 - 6 years of experience in community health/ programme management/ healthcare operations, with at least 2 years in a leadership or supervisory role
- Strong strategic planning, project management, and stakeholder engagement skills
- Proven ability to lead teams, coordinate multi-stakeholder initiatives, and drive measurable community health outcomes
- Excellent communication, presentation, and interpersonal skills
- Ability to analyse data, evaluate programme effectiveness, and recommend improvements
- Proactive, adaptable, and committed to advancing community health and integrated care initiatives
