What you'll be doing
- Build and maintain strategic relationships with clients and community partners to meet residents health and care needs
- Connect resources and stakeholders to enhance primary care capabilities and problem-solving
- Manage partner databases and alliances to ensure continuity and effectiveness of collaborations
- Plan and implement collaborative programmes, including shared care models and community initiatives
- Improve service access for primary care partners, including direct, shared, and community care services
- Support quality improvement frameworks aligned with preventive care objectives
- Identify opportunities for capability development and facilitate knowledge-sharing platforms
- Promote continuous process improvement and programme reviews to meet organisational and community goals
- Support new programme or clinic developments, ensuring operational readiness
- Respond promptly to operational issues or critical situations
- Perform tasks and projects as directed by the Supervisor or Director
What we're looking for
- Bachelor Degree level in Healthcare Management/ Public Health/ Social Sciences or related healthcare studies
- 2 - 5 years of working experience in primary care engagement/ healthcare partnerships/ programme management, preferably in community/ healthcare/ social services setting
- Strong stakeholder management, communication, and coordination skills
- Experience in planning, implementing, or evaluating healthcare programmes preferred
- Able to work independently, manage multiple priorities, and drive quality improvement initiatives
