Community Care Officer (Elderly Services)

Location: Singapore

Community Care Officers support elderly residents under the Enhanced Home Personal Care (HPC+) framework by conducting outreach, identifying needs, and coordinating care and support services.

Key Responsibilities

- Conduct home visits and basic needs assessments

- Engage residents professionally to understand care and wellbeing needs

- Identify requirements relating to personal care, home support, and daily living

- Coordinate services with internal teams and care providers

- Maintain accurate records and reporting in line with programme standards

- Act as a reliable point of contact within the community

Requirements

- Comfortable with community outreach and home visits

- Compassionate, observant, and responsible

- Strong interpersonal and communication skills

- Able to follow structured care and reporting processes
