JOB DESCRIPTION
- Manage casework processes from admission to discharge, including assessments, care planning and documentation.
- Complete required assessments on residents and families using appropriate tools to inform intervention plans.
- Assess residents needs, strengths and risks develop and review Individualized Care Plans (ICP).
- Provide counselling and interventions for residents and families, including planning and executing counselling sessions.
- Maintain accurate case records, reports and updates in relevant systems (e.g. Case Central).
- Coordinate with internal teams and external agencies to support residents progress and family outcomes.
- Prepare and present reports to relevant authorities (e.g. MSF, Youth Court).
- Participate in case discussions, meetings and supervision sessions.
- Support any additional duties or projects as assigned.
REQUIREMENT
- Degree in related field
- 1-2 years of relevant experience in casework and counselling.
- Strong interest in working with children, youths-at-risk and family.
