The Case Worker supports individuals and families facing social, emotional, and practical challenges. The role involves case management, assessment, intervention, and collaboration with community partners to promote safety, stability, and well-being.
Conduct intake assessments and develop appropriate intervention plans
Provide case management support to individuals and families with complex needs
Coordinate with internal teams and external agencies (e.g. community partners, hospitals, schools)
Conduct interviews, home visits, and follow-ups as required
Maintain accurate case documentation and reports in accordance with organisational guidelines
Participate in case conferences, supervision, and team discussions
Support crisis intervention and safeguarding efforts where necessary
Degree in Social Work, Psychology, Sociology, or a related discipline
Prior experience in casework, social services, or community-based roles preferred
Strong interpersonal, communication, and documentation skills
Ability to manage sensitive cases with professionalism and empathy
Comfortable working in a fast-paced and client-facing environment
Proficient in Microsoft Office and case management systems
This is a 1-year contract position, subject to renewal based on performance and organisational needs
Training and supervision will be provided
