ROLE DESCRIPTION

To manage and support the organization with case management, counselling, intake screening/assessments, financial assistance, means testing, and social work-related programs.

SPECIFIC RESPONSIBILITIES

Conduct intake assessments of clients and family/caregivers interviews with FSS and register them for the Association's programs (Children Education Services and Adult Enhancement Services) and services (Enrichment Programs) appropriate to their needs

Conduct case management using the case management model which encompasses these stages: assessment, planning, intervention, evaluation (assess clients problems and effectively identify specific interventions to enhance clients well-being) and termination

Provide counselling and support to clients, their family members, and caregivers

Conduct Means-test to assess clients eligibility to various assistance schemes and subsidies

Review and revise the Association's financial assistance scheme and create a standard criterion based financial assistance scheme

Compile and maintain clients case history which encompasses report writing, case documentation and recording

Coordinate and execute social work-related programs for the Association

Arrange and execute home visits for clients when necessary.

COMPETENCIES

1) Conduct intake screening/assessment interviews.

2) Maintain clients profile database including data entry (CES and AES).

3) Verification of information provided by new clients.

4) Liaise with social service agencies for referrals.

5) Contribute and assist in the formulation of individual care plans for clients. Journey with the clients.

6) Provide counselling and support to clients, their family members, and caregivers.

DURATION OF TRAINEESHIP/ATTACHMENT OFFERED

Six (6) months

APPROVED TRAINING ALLOWANCE

Fresh Graduates - $2,000

Non-Mature Mid-Career Individuals - $2,500

Mature Mid-Career Individuals - $3,000

EDUCATION QUALIFICATIONS REQUIRED FOR THE TRAINEESHIP ROLE UNDER SGUNITED TRAINEESHIP

This position is open for both recent graduates and mid-career individuals (mature and non mature). Graduates interested in this position should possess a University Degree Qualification. Mid-career individuals from any qualification level can apply.
