Responsibilities
- Assess patients for eligibility and suitability for enrolment in the Transitional Shared Care Programme (TSCP). Collaborate with clinicians to facilitate patient enrolment in the programme. Develop tailored follow-up plans based on individual clinical needs. Monitor patient treatment progress, attendance, and adherence to clinical protocols. Arrange necessary follow-up appointments across various healthcare settings, including cardiology clinics and primary care networks. Ensure smooth transitions of care between specialist and primary care environments. Track patient outcomes and document compliance with treatment protocols. Follow up with patients who miss appointments and implement strategies to improve their compliance. Liaise with healthcare professionals to maintain continuity of care. Maintain accurate patient records and databases for effective case management and service evaluation. Collect, prepare, and submit clinical audit data accurately and timely, including AMI mortality data and related diagnostic variables.
Requirements
- Degree in Science or Nursing. A minimum of 3 years of experience in healthcare settings. Strong verbal and written communication skills. Excellent interpersonal skills for effective collaboration across various staff levels. Demonstrated positive and proactive mindset. Ability to work independently while being a supportive team player.
