Care Coordinator, Home Care (1 Year Contract Buyover)

Contract: 1 Year With 1 mth of completion bonus

Working Hours: Mon-Fri, Office Hours

Job Description

To provide client-centric care coordination, discharge planning, monitoring/evaluation and education for clients and their families on Home Care Services offered.

. Manage walk-in, online and phone enquiries on Health services.

. Understand the needs of customers through the use of screening tools, and promote a suitable package of Health services to customers.

. Collaborate with divisions and external partners on service referrals, submit referral forms and ensure follow-up on action points.

(b) Service Coordination

. Administer the coordination of services requested by clients.

. Ensure proper documentation on the systems and paper filing are completed on a timely basis.

. Work closely with internal stakeholders when applicable to ensure the smooth delivery of service, i.e. Home Personal Care (HPC) service is carried out by Care Associate.

. Assist in appointment scheduling for various Home Care services.

(c) Case Management

. Conduct regular home visits and telephonic follow-ups on a monthly basis to engage and build rapport with clients and their caregivers, and obtain feedback on the services rendered.

. Collaborate with other social and health partners to recommend care programs according to the needs of clients, and assist with appeals submission when applicable.

. Counsel and educate caregivers on providing appropriate care, support and financial assistance.

. Arrange for a reassessment of care plans with clients on a 6-monthly basis. This is to determine if the client is required to continue to receive the service.

. Work closely with the Operations and Nurse Manager on discharging clients from service.

(d) Service Development

. Participate in regular programme development and evaluation in order to meet the needs of an evolving eldercare sector.

(e) Revenue Growth & Service Utilisation

. Support in initiatives to drive revenue growth and enhance service utilisation by identifying opportunities, optimising resources, and implementing strategies to achieve business KPIs and maximise client satisfaction and impact within the zone(s).

Requirements:

. Min Diploma or Advanced Diploma in a healthcare-related discipline or Bachelor Degree in related field
