RESPONSIBILITIES

The Care Coordinator will report to the Head, Caregivers Support Services, which is under the umbrella of Community-Based Support Services (CSS). CSS is the dynamic and nimble service delivery arm in MINDS that response to emerging needs of PWIDs and caregivers in the community.

He/she plays a central role in the national pilot of the Caregiver Support Framework and Person and Familycentred Planning (PFP) for the future of persons with intellectual disabilities (PWIDs) and their caregivers. This role involves engaging, assessing, and supporting PWIDs and their caregivers through structured screening, tailored care planning, service coordination, and ongoing monitoring. The Care Coordinator serves as the primary point of contact for both caregivers and PWIDs, working across social, health, financial, and community domains to ensure holistic support.

A proactive thinker, resourceful and responsible team player, he/she also works closely with colleagues and stakeholders to raise awareness of the importance of PFP and future care planning and ensure the smooth delivery of the programme.

Programme Coordination & Support for Future Care Planning

  • Develop a deep understanding of PWIDs and caregivers current situation, strengths, aspirations, and needs, covering psychosocial, health, and daily living domains.
  • Administer the Caregiver Needs Screening Tool to identify needs and triage caregivers into appropriate support tiers.
  • Codevelop tailored, dyadic care plans that address both PWIDs and caregivers needs in the different domains for future care planning.
  • Facilitate access to services and resources across multiple domains (healthcare, social services, financial schemes, community programmes).
  • Provide handholding support to help PWIDs and caregivers navigate and connect with recommended services.
  • Act as the central liaison between the PWIDs, caregivers, service providers, and other community partners.
  • Coordinate crosssector referrals, including between disability and eldercare services, especially for ageing dyads.

Case Management

  • Provide ongoing followups to monitor wellbeing, service access, and progress of care plans.
  • Conduct periodic screenings to track caregiver coping over time.
  • Track escalations, deescalations, referrals, and service utilisation as part of pilot evaluation.

Record keeping

  • Maintain proper records of all cases for internal case reporting and management
  • Collate PWID & caregiver information and other relevant information for submission to SG Enable (SGE) for reporting and evaluation
  • Ensure that all information is kept confidential, in line with the Personal Data Protection Act (PDPA)

Build Stakeholder Relationship

  • Manage communications and feedback from stakeholders
  • Participate in initiatives to establish cordial working relationships with stakeholders
  • Contribute data and support programme evaluation, including validation of the screening tool.
  • Provide feedback, identify gaps, and suggest improvements to workflows, tools, and processes.
  • Participate in training, communities of practice, and capabilitybuilding initiatives under the pilot.
  • Support cocreation of a scalable caregiver support model for future sector-wide implementation.
  • Assist to integrate pilot processes with existing workflows in DACs and Sheltered Workshops
QUALIFICATIONS
  • Degree in Social Work, Psychology, Counselling, or related social service fields from recognised tertiary institutions
  • Experience in social services, case management, disability services or person-centred planning preferred
OTHER INFORMATION
  • Effective and good interpersonal, communication and time management skills
  • Possess analytical and critical thinking
  • Well organised and detail-oriented
  • Have strong interest and positive attitude towards understanding PWIDs and caregivers needs.
  • Proficient in MS Office and databoard applications will be an advantage
