As a Care Coordinator - Healthcare, you will be responsible for the following duties:
- Provide coordinated, end-to-end care support for patients and clients, including discharge planning, follow-ups, and ongoing monitoring of progress.
- Work closely with internal care teams and external partners to ensure smooth delivery of health and home care services.
- Assess patient and client needs using screening tools and clinical indicators, recommending appropriate care programmes, preventive screenings, vaccinations, and service packages.
- Leverage digital tools and IT systems to document care activities, track outcomes, and support health monitoring (e.g. blood pressure, glucose, weight).
- Deliver health education and caregiver counselling on lifestyle management, social prescribing, home care practices, and available financial or community resources.
- Conduct home visits and telephone follow-ups to engage clients and caregivers, ensuring care plans remain relevant and effective.
- Support initiatives that improve service utilisation, programme development, and overall care quality while contributing to positive clinical and social outcomes.
Requirements:
- Minimum Diploma in Health Promotion or healthcare-related disciplines.
