Care Coordinator (Assisted Daily Living)

The program Care coordinator will visit the homes of residents together with community grassroots to do preliminary assessments of residents who may have difficulties in performing Assisted Daily Living activities and make referrals for a short-term rehabilitative stay at Cheshire Home for training and support.

Job Description:

• Work closely with stakeholders to ensure the smooth delivery of services
• Manage inquiries on care services and coordinate requested services
• Assess customer needs using screening tools and recommend suitable service
• Collaborate with multi-disciplinary teams and external partners on service referrals, submit referral forms, and ensure follow-up on action points
• Conduct regular home visits and telephone follow-ups to engage with clients
• Collaborate with social and health partners to recommend appropriate care programs
• Counsel and educate caregivers on providing effective care, support, and available financial assistance
• Work closely with internal team on client discharge from services

Requirement:

Possess a Diploma in healthcare-related disciplines

Have a minimum 2 years experience in community healthcare settings, Senior Care Centers or Family Service Centers.

Previous training/understand basic nursing procedures or therapy needs assessment would be advantageous.

Strong administrative and coordination capabilities with Cheshire's rehab therapist, nursing, social worker and psychologist teams for subsequent visitations

Proficient in Microsoft office

Can speak dialects or more than one language.
