We are seeking passionate and driven individuals to join the Precinct Planning & Development (PP&D) team. Reporting to a Manager or Senior Manager, you will play an important role in planning and implementing precinct-level strategies, strengthening partnerships with community, healthcare and government partners to improve population health and well-being, coordinating multi-stakeholder initiatives, and translating insights into action to create healthier communities. Job responsibilities

1. Support Healthy Precinct Planning

• Conduct population, community and precinct analysis to identify population health needs and opportunities.
• Develop and support the development and implementation of Community Health Plans.
• Translate population health priorities into practical initiatives and workplans for assigned precincts.
• Support the development of theories of change, logic models and implementation plans for community-based interventions.

2. Build and Maintain Community Partnerships

• Build positive working relationships with healthcare providers, social service agencies, grassroots organisations, community groups and government partners.
• Drive and/or support multi-stakeholder workgroups and partnership platforms.
• Engage stakeholders to strengthen collaboration and collective action towards shared population health and community goals.
• Participate in resident engagement and co-creation activities to ensure initiatives remain responsive to local needs.

3. Lead Precinct Programmes and Projects

• Lead the planning and implementation of Healthy Precinct initiatives across assigned precincts with multiple stakeholders and agencies.
• Monitor project milestones, deliverables and risks, and support follow-up actions.
• Track and report project progress and outcomes.

4. Support Data-Informed Planning and Evaluation

• Gather, analyse and interpret population health, programme and community data.
• Track programme implementation and outcomes to support continuous improvement.
• Develop dashboards, reports and presentations for internal and external stakeholders.

5. Support Governance, Reporting and Communications

• Prepare meeting materials, presentations, reports and briefing papers for Healthy Precinct platforms, senior management and/or partner organisations.
• Document key discussions, decisions, action items and follow-up plans.
• Facilitate effective communication and coordination across multiple stakeholders.

Requirements

• A bachelor's degree in any field.
• At least 3 years of relevant experience in stakeholder engagement, programme management, partnership development, community development, policy, strategy or project management.
• Strong interpersonal, facilitation and relationship-building skills.
• Strong analytical, problem-solving and organisational abilities.
• Excellent written, verbal and presentation skills.
• Ability to work collaboratively across multiple stakeholders and disciplines.
• Comfortable navigating complexity and managing competing priorities.

• Experience in Singapore healthcare system, multi-agency or cross-sector partnerships, or strategy development and implementation will be advantageous
