About the Role

We are looking for an Assistant Manager to support the strategic planning, development and implementation of the Integrated Community Care System (ICCS) and initiatives initiated by MOH.

You will manage key workstreams that bring population health strategies into practice, including primary and community care integration, service and care model development. This role offers the opportunity to work across strategy, programme design and ground implementation with diverse stakeholders within healthcare system, government agencies and social care partners.

Key Responsibilities

Strategic Planning & Programme Development

  • Support the development and implementation of Integrated Community Care System and Age Well Neighbourhood strategies and workplans.
  • Translate strategic priorities into practical care models, service pathways, workflows and implementation plans.
  • Conduct analysis and identify opportunities to improve integration and delivery of health and social care in the community.

Project & Workstream Management

  • Independently manage assigned workstreams from planning and design to implementation and evaluation.
  • Develop and track project plans, milestones, deliverables and performance indicators, and proactively address implementation issues.
  • Key workstreams may include primary-community care integration, enhanced Community Health Posts in Age Well Neighborhood, care model development for frailty, chronic disease management, strategies for community outreach and other population health initiatives.

Stakeholder & Partnership Management

  • Work closely with internal and external stakeholders e.g. GPs, polyclinics, Community Care Teams, Active Ageing Centres, social care providers and other partners to co-develop and implement integrated care initiatives.
  • Facilitate stakeholder discussions to align objectives, roles, referral pathways and operational workflows.
  • Build strong working relationships across healthcare and community partners to drive implementation.

Performance & Evaluation

  • Monitor programme implementation through appropriate indicators and dashboards.
  • Analyse data and ground feedback to identify gaps, evaluate effectiveness and recommend programme improvements.
  • Support management reporting and evaluation of pilots and new care models.
  • Prepare management presentations, proposals, progress updates and recommendations to support decision-making.
  • Coordinate relevant workgroups, ensuring key decisions and follow-up actions are translated into implementation.

Job Requirements

  • Bachelor's degree in Healthcare Management, Public Health, Business, Public Policy, Social Sciences or a related discipline is preferred
  • Preferably at least 4-5 years of relevant experience in healthcare, population health, community care, public sector, consulting, programme management or related fields.
  • Experience in strategic planning, project/programme management, service development or healthcare operations would be advantageous.
  • Strong project management, analytical and problem-solving skills, with the ability to independently manage multiple workstreams.
  • Strong stakeholder management and communication skills, with the ability to work effectively across diverse healthcare and community partners.
  • Able to translate complex or ambiguous issues into practical plans and solutions.
  • Resourceful, organised and proactive, and comfortable working in a dynamic and evolving environment.
