Job Description

Case Referrals & Homecare Coordination

  • Stay updated on policies and support Homecare service development.
  • Handle case referrals, coordination, and all client registration/admission/discharge procedures.
  • Manage NMTS and SMF applications and ensure timely renewals.
  • Maintain accurate client databases and conduct satisfaction surveys.
  • Collate reports, support KPI and audit requirements, and build rapport with AIC/MOH.

Administrative & Operational Support

  • Provide admin, procurement, billing, and logistical support for the Homecare team.
  • Manage documentation, filing, scheduling, minutes, and incoming/outgoing correspondence.
  • Ensure proper upkeep of client records and operational files.

General & Other Duties

  • Manage front desk duties and handle calls and walk-in enquiries.
  • Support outreach events, ad-hoc projects, renovations, and data entry.
  • Perform any additional duties as assigned.

Requirements:

Qualifications & Experience

  • Diploma in Administration, Social Services, Healthcare, or relevant fields.
  • Experience in case coordination, administrative support, or healthcare operations is an advantage.
  • Familiarity with systems (e.g., IRMS, NMTS, ILTC, EMRX) and homecare processes preferred.

Skills & Competencies

  • Strong organisational and administrative skills, with attention to detail.
  • Good communication and interpersonal skills to liaise with clients, caregivers, AIC, MOH, and internal teams.
  • Ability to maintain accurate records, manage databases, and handle confidential information responsibly.
  • Proficient in Microsoft Office and basic IT systems able to prepare reports and documentation.
  • Able to multitask, prioritise workload, and support team operations smoothly.
  • Customer-service oriented, with a professional and friendly disposition.

  • Responsible, reliable, and able to work independently with minimal supervision.
  • Adaptable and able to handle ad-hoc tasks or operational changes.
  • Team player with a positive attitude and willingness to assist in outreach or projects when needed.
