Community Support: CCMS (Community Case Management Services) | CREST (Community Resource, Engagement and Support Team) | Henderson Home

RESPONSIBILITIES

• Conduct/provide support on active ageing programmes
• Encourage active participation with seniors
• Provide support to seniors at outreach events and programmes
• Provide support for outreach efforts to seniors at their homes
• Carry out administrative functions such as data entry and record keeping
• Conduct/provide support on the creation of social media content regarding Active Ageing Centres
• May be required to perform additional duties, work over the weekends or after office hours

• Good judgment, problem-solving, and decision-making skills
• Independence and accountability, with the ability to meet deadlines
• Proficiency in Google Workspace applications
