Virtual Nurse

Responsibilities

Review hospital referrals and assess patient eligibility for SNF admission.

Utilize the Red-Yellow-Green clinical system to determine if a patient meets facility requirements.

Collaborate with the Admissions Coordinator and Financial Coordinator to complete the admission process.

Communicate with hospital case managers as needed regarding clinical assessments.

Work closely with other nurses to ensure accurate and timely evaluations.

Maintain up-to-date knowledge of facility capabilities and update guidelines accordingly.

Provide virtual clinical assessments based on hospital documentation (no hands-on patient care required).

Be flexible in covering multiple locations/islands if needed.

Qualifications and Requirements

Experience in skilled nursing, long-term care, or hospital settings.

Familiarity with clinical triage processes and assessment tools.

Experience working with electronic medical records (EMR) systems.

Ability to work independently in a remote setting.

Strong analytical and decision-making skills in clinical assessments.
