URGENT Senior Social Worker - 5 Day Office Hours HRY

. Conduct psychosocial and financial assessments

. Provide direct counselling, emotional support, and psychoeducation

. Act as liaison for patients/clients and their families

. Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans

. Actively participate in and contribute to departmental and organizational quality improvement initiatives

Qualifications, Competencies & Experience

• Master/Bachelor/Post Graduate Diploma in Social Work

• Registered Social Worker with the Singapore Association of Social Workers (SASW)

• Minimum 3 years of relevant working experience in Social Work sector and eldercare setting

• Family Service Centre (FSC) experience is required
