What You'll Be Doing
- Conduct intake assessments and regular reviews of day care and rehab clients, facilitating multi-disciplinary meetings for client care planning
- Supervise daily routines, including meal preparation, activities, and personal care assistance, ensuring the well-being of all clients
- Collaborate with the centre nurse and care staff to address clients specific needs and ensure proper care delivery
- Plan and coordinate programmes and services with both internal teams and external stakeholders to enhance client engagement
- Provide guidance and ongoing training to new and existing care staff, ensuring quality care for both day care clients and those with dementia
- Oversee daily operations, staff deployment, and safety measures to maintain an effective and secure care environment
- Manage and resolve incidents involving staff, clients, and volunteers, ensuring proper reporting and follow-ups
- Work with the centralised team on client billing, charges, and staff-related administrative matters
- Support financial sustainability efforts by working closely with the cluster lead on budget planning and resource allocation
- Ensure accurate and timely documentation of assessments, client communications, and operational processes
- Perform additional duties as assigned to enhance service quality and operational efficiency
What We're Looking For
- Bachelor Degree in Healthcare Management/ Public Health or related studies
- 5 - 6 years of experience in clinical area such as acute or community hospital, preferably with 2 years of managerial role
- Good knowledge of the health care environment and service processes is important
- Experience in community care provision is an added advantage.
- Integrity, highly motivated, a team leader and able to maintain good interpersonal relationships, in collaboration with various partners in the healthcare system
