- Provide a high standard of medical care and be part of the teaching faculty of the hospital.
Main Duties & Responsibilities
General Professional Duties
- Participate and contribute towards teaching and training sessions.
- Participate in research work under the supervision of senior doctors (optional).
- Participate in family conferences for patients looked after.
- Participate in committee work, project(s) as directed by the Medical Director.
- Participate in selected incident review(s) as directed by the Medical Director.
Site Specific Medical Duties (Community Hospital)
- Carry out daily clinical rounds in assigned area/s by reviewing patients condition and ordering treatment and investigations as required.
- Attend to new admissions or referrals by reviewing patients referral notes, performing a comprehensive history, performing physical examinations and interviewing family members (as appropriate).
- Carry out rounds with visiting consultants, and to consider/carry out their recommendations as appropriate.
- Perform clinical procedures e.g. suturing, venepuncture, setting up IV lines, catheterization.
- Participate in Patient Care Conference (also known as multidisciplinary meetings), and contribute to the multidisciplinary care and/or discharge planning of patients. This may include screening and referring patients to other services such as specialist, senior care centres, day rehabilitation centres, Home Nursing Services and/or Home Medical Services.
- Attend to emergencies in the assigned area/s.
- Do exit rounds (in-patient setting) and to hand over to the doctor on-call.
- Do 1st Line on-call (sleep-in) or stay-out calls as scheduled.
Site Specific Medical Duties (Nursing Homes)
- Attend to clinical needs of residents in the nursing home for a stipulated duration through regular rounds, review of conditions and ordering treatment and management as required.
- Clerk and perform comprehensive geriatric assessment for new admissions to the nursing homes.
- Lead and engage with the multidisciplinary team members during patient care conferences to optimize care plan for resident.
- Review patients after discharge from acute admissions and to ensure medication list has been updated.
- Perform suitable clinical procedures within the nursing home setting.
Site Specific Medical Duties (Home Care)
- Review and look after patients in the home care setting to maintain continuity of care for chronic medical conditions in the home setting.
- Respond to requests for ad hoc home visit for patients looked after.
- Perform comprehensive geriatric assessment for all new home care patients with careful review of medications and social/ functional needs.
Site Specific Medical Duties (Outpatient Clinics)
- Manage acute and chronic medical conditions for patients seen at the outpatient clinics.
- Familiarity with Healthier SG initiatives and to offer the relevant updated screening and preventive care for patients seen.
Qualification
- Medical degree that is recognized by the Singapore Medical Council.
- Basic Post-graduate medical degree acceptable as specialist qualification, e.g. Member of Royal College of Physician and Masters in Family Medicine (Family Physician).
- A minimum of 2 years post qualification experience.
- Has experience and passion in clinical work.Is a person of integrity, highly motivated and able to maintain good interpersonal relationships and work in collaboration with the multi-disciplinary team.
