As a member of the Singapore Hospice Council and Asia Pacific Hospice Network, we provide free, home-based palliative care focused on dignity, comfort, and holistic support for both patients and families.

What You'll Do

As a Staff Nurse, you'll bring holistic, person-centred nursing to patients homes-ensuring physical comfort, emotional support, and dignified end-of-life care.

Key Responsibilities

• Conduct home visits and palliative nursing assessments
• Develop and review individualised care plans to manage symptoms effectively
• Administer medications and adjust care based on doctor's directions
• Educate and support caregivers in providing end-of-life care
• Coordinate referrals to inpatient hospices or community services
• Offer emotional and bereavement support to families
• Maintain accurate documentation target minimum of 6 visits/day
• Support outreach and fundraising efforts when needed

What You Bring

• Registered Nurse with SNB (Diploma/Degree in Nursing)
• Palliative Care certification is a plus
• Minimum 3 years of nursing experience (community/hospice preferred)
• Warm, empathetic, and professional - able to support families with sensitivity
• Comfortable with independent home visits
