Job Description

The Staff Nurse will be responsible for delivering person-centred nursing care, coordinating care plans, and supporting residents health and well-being in accordance with established nursing standards and evidence-based practices. The role involves working closely with multidisciplinary teams, residents, families, and community partners to ensure quality care and positive health outcomes.

Key Responsibilities:

  • Conduct comprehensive nursing assessments and develop individualized care plans based on residents needs and preferences.
  • Deliver evidence-based nursing care and administer medications in accordance with organisational policies and protocols.
  • Monitor residents health conditions, identify changes, and escalate concerns appropriately.
  • Maintain accurate and timely clinical documentation and nursing records.
  • Facilitate care planning discussions and support Advanced Care Planning initiatives.
  • Build therapeutic relationships with residents, families, caregivers, and community partners.
  • Provide health education, training, and support to residents and caregivers to promote self-management and well-being.
  • Coordinate care transitions and referrals in collaboration with multidisciplinary healthcare teams.
  • Support residents and families during transitions between different care settings.
  • Participate in quality improvement initiatives, audits, and evidence-based practice projects.
  • Ensure compliance with infection control, workplace safety, and risk management protocols.
  • Identify and report risks, incidents, and safety concerns in accordance with organisational guidelines.
  • Support emergency preparedness and respond appropriately during public health emergencies.
  • Participate in continuing professional development, training programmes, and nursing education activities.
  • Contribute to a positive learning environment and support the professional development of the nursing team.

Requirements:

  • Diploma or Degree in Nursing.
  • Registered with the Singapore Nursing Board (SNB) and holding a valid practising certificate.
  • Valid BCLS + AED certification.
  • Minimum 1-3 years of relevant nursing experience in a nursing home, community care, rehabilitation, or acute hospital setting.
  • Candidates without Singapore experience may be considered and will undergo an intensive induction programme.
  • Strong clinical assessment, patient care, and care coordination skills.
  • Good communication and interpersonal skills with the ability to work effectively with residents, families, and healthcare professionals.
  • Familiarity with nursing standards, infection control practices, and patient safety protocols.
  • Basic IT proficiency and ability to maintain electronic records.
  • Willing to work rotating shifts, weekends, and public holidays when required.
