Social Worker / Social Work Executive (Project JOI) - One Year Contract

Overview
Project JOI (Journey of Integration) builds on the success of Heads-Upp, a pioneering social-health integration initiative that has demonstrated the impact of bringing healthcare and social support together for young children and families.

As the next phase of this work, Project JOI expands this integration across social, health, and early childhood systems, and brings the model into a new region in the North-East, in partnership with healthcare and early childhood providers. This role sits at the heart of that integration.

As a Social Health Worker / Integrated Care Practitioner, you will work with families not just at the level of individual needs, but across the parent-child relationship, family environment, and preschool setting. You will partner closely with healthcare professionals, educators, and community services to deliver coordinated, family-centred care.

This is a 2-3 year pilot role for those who are keen to be part of building and shaping an emerging model of care-where your work contributes directly to how integrated, preventive support for children and families is designed and delivered in Singapore.

Integrated Care Practitioner (Social-Health) Job responsibilities:

  • Understand the Whole Child and Family Context: Engage families through home visits and other touchpoints, conducting holistic assessments that consider child development, caregiver wellbeing, family dynamics, and environmental factors.
  • Develop and Implement Integrated Care Plans: Design and carry out child- and family-centred care plans that integrate social support, developmental guidance, and health-promoting strategies.
  • Coordinate Across Systems and Enable Access: Work closely with multidisciplinary partners-including healthcare teams, early childhood educators, allied health professionals, and community agencies-to align interventions, facilitate warm referrals, and support families in navigating services.
  • Build Caregiver Capability and Confidence: Provide psychoeducation, coaching, and practical support to strengthen parenting skills, responsive caregiving, and health-promoting routines.
  • Safeguard Well-Being: Monitor risks and respond to emerging concerns, while maintaining accurate documentation.
  • Contribute to Programme Learning: Implement and support programme evaluation, learning, and continuous improvement.

Requirements:

  • Degree in Social Work or Early Childhood Education, Psychology preferred.

  • Candidates with no experience but with relevant qualification will be considered and relevant training, internships, or practicum experience in social services, early childhood, healthcare or community-based settings will be an advantage.

  • Strong interest in participating in a pilot programme and contributing to the development and refinement of new service models.

  • Deep interest in systems collaboration and cross-sector work involving social, health, and early childhood partners.

  • Willingness to learn, adapt, and work in an evolving, flexible programme environment.

  • Good interpersonal and communication skills, with the ability to engage families and work with multidisciplinary partners.

  • Being proficient in a second language will be an advantage.

  • Proactive, reflective, and committed to improving outcomes for children and families.

  • Comfortable with occasional night or weekend visits, and travelling to work across different Care Corner offices or programme locations, especially to North-East regions as required.
