Social Worker (FSC Punggol)

Roles & Responsibilities:

CASEWORK AND COUNSELLING

. Manage 25-30 C&C active cases per quarter.

. Conduct assessment, planning and intervention for cases by taking into account the integration of a range of information, including practice guides and organisational frameworks.

. Conduct case conferences with partners.

. Participate in networking and case management meetings.

. Document and evaluate case outcomes.

. Develop and review appropriate intervention strategies.

. Ensure proper documentation of cases.

. Ensure reports on cases are submitted in a timely manner.

INFORMATION AND TRIAGE

. Manage case referrals.

. Part of the Intake & Triage Team to conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.

. Refer clients to appropriate agencies for further intervention.

COMMUNITY WORK

. Network with other organizations and stakeholders.

. Participate in implementing and evaluating of community outreach / initiatives.

GROUP WORK

. Be part of the programme team for Group Work conducted at the FSC.

REQUIREMENTS

• Degree or Graduate Diploma in Social Work
• Previous experiences via internships or placements in related areas of work will be considered
• Understanding of social work values and ethics
• Understanding of work with vulnerable populations i.e. low-income families
