Social Worker (Senior Care Center)

1.JOB PURPOSE

The jobholder is responsible in assessment, intervention and care for the seniors.

2. DUTIES AND RESPONSIBILITIES

2.1 Primary Responsibilities and Duties (90%)

2.1.1 Provide psycho-social support for patients and their families through case management, counselling, financial assessment and recommend resources where necessary.

2.1.2 Participate in the development of the care plan and assist staff members in understanding the significant social and emotional factors related to clients health problems.

2.1.3 Collaborate with other healthcare professionals and family members/caregivers to determine appropriate care plan.

2.1.4 Experiences in claiming of Medifund will be at advantage plus.

2.2 Secondary Responsibilities and duties (10%)

2.2.1 Any other ad hoc projects as assigned by supervisor.

3. JOB SPECIFICATIONS/REQUIREMENTS

3.1 Qualifications and Experience

3.1.1. Diploma/Degree in Social Work.

3.1.2. Minimum 2 years working experience in healthcare/ILTC sector preferred.

3.1.3. Familiar with health care system to liaise with various government agencies preferred.

3.2 Abilities and Attributes

3.2.1. Ability to speak local dialects is necessary.

3.2.2. Committed to social vision and attend to client's needs.

3.2.3. Independent, resourceful and able to perform under pressure.
