Job purpose
The Social Worker will provide support in case managementand groupwork at SASCO@Telok Blangah and SASCO@West Coast Senior Care Centres(SCCs). The role ensures consistent and quality social work practice, andalignment with organisational and regulatory standards across the cluster.
Key responsibilities
1. Case Management
- Review referrals from the Agency for Integrated Care (AIC) and conduct intake assessments for seniors admitted to SCCs.
- Provide holistic case management, including comprehensive biopsychosocial and risk assessments for seniors and their families.
- Develop, implement, and review Individual Care Plans (ICPs) in collaboration with the multidisciplinary team (MDT) to support clients needs and enhance their quality of life.
- Provide psychosocial interventions, basic counselling, and caregiver support as required.
- Conduct financial assessments and facilitate applications for financial assistance for eligible seniors.
2. Practice Quality
- Participate actively in case discussions, case conferences, group work and multidisciplinary collaborations with internal and external stakeholders.
3. Practice Governance & Quality Assurance
- Ensure compliance with agency policies, professional codes of ethics, and regulatory requirements.
- Maintain accurate and timely case documentation in accordance with agency and AIC standards.
- Contribute to the development, refinement, and implementation of SOPs, workflows, and practice guidelines across the cluster.
- Any other tasks as assigned
Qualifications and Skills Required
- Degree in Social Work recognised by the Singapore Association of Social Workers (SASW)
- RSW is a requirement
- Minimum 1-2 years of social work practice experience.
- Experience in casework, risk management, and multi-stakeholder collaboration.
- Strong clinical judgement and ethical reasoning.
- Ability to anchor, influence, and support practice across multiple centres.
- Good administrative and organizational skills
