RESPONSIBILITIES

About the Role

The Integrated Community Care Provider (ICCP) brings together community care providers and partners to strengthen coordination and integration of health and social care within the community.

As a Social Worker, you will support the coordination and implementation of ICCP initiatives, working with partner organisations, community care teams, healthcare professionals, and community stakeholders. The role also includes casework and participation in other programmes or projects based on professional competencies and organisational needs.

Role & Responsibilities

ICCP Coordination, Partnership & Administration

  • Coordinate ICCP workplans, initiatives and deliverables across partner organisations.
  • Organise and facilitate meetings, check-ins, and engagement activities to strengthen partnerships and collaboration.
  • Develop and maintain networks with healthcare, community care, grassroots, and other relevant partners.
  • Represent the subregion at relevant meetings and communicate key updates and follow-up actions.
  • Monitor programme progress, indicators, and deliverables, and follow up on agreed actions.
  • Collect and consolidate information and prepare programme reports and other relevant documentation.
  • Maintain accurate, and organised programme and case documentation.

Casework & Service Delivery

  • Conduct triage, intake, and bio-psycho-social assessments to identify clients needs, strengths, and risks.
  • Provide case management and develop person-centred care plans for seniors and their caregivers.
  • Conduct home visits and implement appropriate casework and psychosocial interventions, and monitor clients progress and outcomes.
  • Collaborate with caregivers, healthcare professionals, and community partners to coordinate care and facilitate access to services.
  • Support supervised sessions under the Supervised Access Programme for Vulnerable Adults, where required and based on competency.
  • Contribute to other programmes, projects, or service initiatives according to organisational needs and professional expertise.
  • Ensure timely and accurate case documentation and reporting.
  • Participate in supervision, team meetings, training, and other professional development activities.
QUALIFICATIONS
  • Degree, graduate diploma, or professional qualification in Social Work, Counselling, Psychology, or a related discipline.
  • Open to new graduates and early-career professionals with 1 to 2 years of relevant work.
  • Genuine interest in working with seniors and supporting caregivers in a community-based setting.
  • Strong interpersonal, communication, organisational, and problem-solving skills.
  • Able to work independently while contributing to a collaborative team environment.
  • Proficient in written and spoken English fluency in a local Mother Tongue language and/or dialects is an advantage.
  • Comfortable with casework or direct service delivery and able to manage client-related responsibilities within the scope of professional competency.
  • Committed to professional and ethical practice, continuous learning, and reflective practice.
