Job Description

Brief Summary

Responsibilities

  • Conduct casework and deliver psychosocial support to patients, caregivers, and families. Review intervention plans and collaborate with all relevant stakeholders, including external agencies. Manage patient admissions and perform assessments for financial assistance. Plan, organise, and oversee social programmes tailored for patients. Maintain and update patient records and billing information. Lead and facilitate group work and programmes designed for targeted client groups. Promote services to client groups and advise stakeholders on relevant programmes and services.

Requirements

  • Degree in social work with accreditation. Minimum of 2 to 3 years of relevant experience. Strong dedication to improving patients lives through holistic social care. Excellent teamwork abilities, along with effective written communication and interpersonal skills.
