Job Description :
- Create personalised care plans tailored to the needs of the resident
- Implement care plans and regularly record residents progress
- Design and organise programs and activities to address residents care requirements
- Prepare and submit review reports and monthly interview records every 6 month
- Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents
Job Requirement :
- Min Diploma/Degree in Social Work/Psychology/Counselling
- At least 1 year of relevant healthcare industry experience
- Good interpersonal and problem-solving skills
- Proficient in English and able to converse in dialects or other languages
- Office Hours: 5 days work week
